Standalone Document

This disclosure is provided to you as a standalone document, separate from the employment application and any other document. You will be asked to sign a separate authorization form before any consumer report is procured. Please read this disclosure carefully and ask any questions before proceeding.

1. Disclosure

Jet Set Line, Inc. ("Company") may obtain information about you from a consumer reporting agency for employment purposes. This information may be obtained in the form of a "consumer report" or an "investigative consumer report" within the meaning of the federal Fair Credit Reporting Act, 15 U.S.C. §1681 et seq. ("FCRA").

These reports may include, but are not limited to, the following information: identity verification (Social Security Number trace), criminal history (federal, state, and county), motor vehicle records ("MVR"), prior employment verification, education verification, professional license verification, reference checks, and drug-test results (where required by federal motor-carrier regulations).

The reports will be prepared and furnished to Company by a consumer reporting agency. Company's current consumer reporting agency is identified in the separate Authorization form provided to you. You may request the name, address, and telephone number of the consumer reporting agency and the nature and scope of any investigative consumer report by contacting hr@jetsetusa.com.

2. Your Rights Under the FCRA

You have rights under the FCRA, including the right to know the nature and scope of any investigative consumer report no later than five (5) business days after Company's receipt of any written request from you. A summary of your rights under the FCRA, titled "A Summary of Your Rights Under the Fair Credit Reporting Act," will be provided to you separately. You may also obtain the current summary from the Consumer Financial Protection Bureau at consumerfinance.gov/learnmore.

3. Pre-Adverse and Adverse Action Procedures

If Company intends to take any "adverse action" (including denying employment, withdrawing a conditional offer, or terminating employment) based in whole or in part on information contained in a consumer report, Company will:

— Provide you with a copy of the report;

— Provide you with a copy of "A Summary of Your Rights Under the Fair Credit Reporting Act";

— Allow you a reasonable period of time (at least five business days) to review and respond, including the opportunity to dispute the accuracy or completeness of the report directly with the consumer reporting agency; and

— If a final adverse action is taken, provide an Adverse Action Notice that includes the name, address, and telephone number of the consumer reporting agency, a statement that the agency did not make the decision and cannot explain why it was made, and notice of your right to obtain a free copy of the report and dispute its accuracy.

4. State-Specific Notices

Additional state-specific notices may be provided to you depending on your state of residence (including, where applicable, California, New York, Massachusetts, Maine, Minnesota, Oklahoma, Washington, and New Jersey). Where state law conflicts with this disclosure, state law controls to the extent of the conflict.

5. Authorization (Separate Form)

A separate Authorization form will be provided to you, which you must sign before Company will obtain any consumer report. The Authorization is a separate document and is not part of this Disclosure. Refusal to sign the Authorization may result in withdrawal of any conditional job offer or termination of the application process. You may withdraw your authorization at any time by written notice to hr@jetsetusa.com, except that withdrawal does not affect actions taken in reliance on prior authorization.
